The prenatal vitamins her adoptive sister, Gianna Lazzari, had handed her tore through her stomach in a deep, twisting cramp that refused to ease.
Her eldest brother, Alessio Lazzari, the most skilled surgeon the Lazzari family had, said she needed an emergency C-section immediately.
Her mafia husband, Rafe Bellandi, completely lost composure. “No. Just keep Nina alive!”
Even through the agony, she felt fortunate. In a world this ruthless, everyone she loved had still shown up for her.
Then, while she waited for the anesthesia to take effect, she caught their voices through the door.
There was something sharp in Alessio’s tone, almost like excitement bleeding through.
“Nina’s healthy. Her blood type’s a match, her age works, everything lines up perfectly. We won’t get another shot like this. We need to give her heart to Gianna.”
Matteo, the Don of the entire Lazzari family, didn’t hesitate for a second.
“I’ll erase the records. Nina will never know.”
Dario, their brilliant pharmacology professor, one of the best in New York, agreed instantly. “I’ll create a custom anti-rejection formula so her body doesn’t fight the new heart. She won’t feel anything.”
A heavy pause followed. Then Rafe spoke, his voice thick with a disturbing kind of relief.
“Gianna needs a healthy heart if she’s ever going to dance again. Nina… we’ll find a way to make this right.” He let out a breath. “I’ll sign the consent.”
Her entire body turned to ice.
Her chest already throbbed as if it had been split open, as if the heart inside her was already gone.
Then a voice cut through the fog, one she almost recognized.
[Nina! You’re seconds from dying! Are you really willing to give up everything, every single reward, just to keep existing in this world?]
She laughed. Or maybe she sobbed. Possibly both at once.
Not this time. Their twisted definition of love wasn’t enough to keep her anchored here anymore.
She tried to roll away, to pull herself off the bed, but a nurse was already pressing a breathing mask to her face and forcing an IV into her arm.
Everything happened in seconds.
The anesthesia hit her bloodstream like a speeding train.
But just before darkness swallowed her, she watched them wheel Gianna into the room. The adopted daughter everyone had spoiled beyond reason. The one who had poisoned her and ended her pregnancy.
Alessio already had the scalpel ready. His voice turned to ice as he addressed the nurse.
“Forget the fetus. Take Nina’s heart first. Gianna’s transplant comes before anything else.”
The blade sank into her chest. She could swear she felt every single layer it sliced through.
Alessio’s voice reached her right before consciousness slipped away, barely above a whisper.
“Hold on, Gianna. That stage is yours the moment you wake up.”
From whatever fragment of awareness remained, she answered the voice in her head.
[If this body dies, I return to my real world, right? My heart gets repaired, and I still get the hundred million dollars?]
[Yes.]
What a joke.
She was never meant to stay here in the first place.
The only reason she’d entered this world was desperation, unable to afford her own heart surgery back home. So she’d taken the deal: come here, protect Alessio, Matteo, Dario, and Rafe from the ruin waiting for them.
Fifteen years she’d spent keeping them safe. In return, they’d carved her open to save someone else.
She yanked the oxygen cannula from her nose.
Her throat sealed shut instantly. Every breath became a battle her body was losing. Her damaged heart couldn’t keep pace. Her lungs burned, and no matter how hard she tried, no air would come.
Her face turned a deep shade of purple. She had seconds left.
But she was smiling.
Dying felt fine to her. Far better than staying here, drowning in their lies.
The heart monitor let out one long, screaming flatline tone, red lights flashing through the room. Alessio, Matteo, Dario, and Rafe burst through the door, a team of nurses close behind.
All four men shouted at once. “Nina!”
Alessio performed compressions until her heart restarted. He pressed the oxygen mask back over her face, his eyes rimmed red when he finally looked at her.
“What is wrong with you? Have you lost your mind? You pulled out your own oxygen line. It was a miscarriage, Nina. That’s not something worth dying for!”
She pressed her palm against the heavy bandage over her chest, pain blinding her, and forced the words out raw.
“Is the baby… the only thing I lost?”
