Rafe stared at her, guilt visibly tearing him apart inside.
“This one’s on me, Nina. I’m the one who told Gianna to bring you those vitamins.”
“I thought they’d help protect the baby. I had no idea you’d react this badly. I never imagined it would—” His voice cracked. “God, the toxins went straight for your heart.”
“But Alessio’s the best cardiac surgeon in the entire country. He’ll fix this. And we can try for more kids, Nina, I swear it.”
He moved closer to the bed, his eyes swollen and bloodshot, wrapping both hands around hers.
“The doctor who wrote that prescription? Already taken care of. Her license was revoked, and she’s been driven out of the city completely. She’s finished in the medical world. Don’t blame Gianna for this. Don’t blame yourself either. You have people who love you, Nina.”
She said nothing. Matteo cleared his throat, his tone firm.
“He’s right, Nina. We’ve already contacted every transplant program available, domestic and international, all of them. We will get you a new heart, no matter what it costs.”
Dario squeezed her shoulder gently, as if she might shatter at the slightest pressure.
“Don’t stress, princess. I’m going to formulate something specifically for you, something that’ll have you back to normal in no time. Running the estate, handling everything, all of it.”
All she felt was a hollow chill.
The sheer nerve of it all turned her stomach. A handful of soft words, and they actually believed that would erase everything? Gianna had poisoned her, ended her pregnancy, and stolen her heart.
She looked around at the four of them gathered at her bedside, a cold, humorless smile forming.
“Where’s Gianna? I want to hear her side of this myself.”
Alessio’s jaw clenched.
“You’re still blaming Gianna? The guilt hit her so hard it triggered a severe heart attack. They barely located a donor heart in time to save her.”
Dario cut in before she could respond.
“Just concentrate on your recovery, alright? If Alessio managed to save Gianna, he can do the same for you. It’s only a matter of time.”
She let out a sharp, breathless laugh.
“What a coincidence. My heart fails on that exact operating table, and Gianna conveniently finds a donor just down the hallway?”
Silence dropped over the room instantly.
“Nina—” Alessio started, then caught himself, shifting under her stare.
She was drowning in their lies.
She forced a thin, lifeless smile.
“Relax, I’m joking. Go check on Gianna. I’m so happy she’s recovering well.”
The three men exchanged uneasy looks, lips parting as if to argue, then thinking better of it.
She turned away from them, offering nothing but cold silence.
When she looked back, they had already left.
Only Rafe remained, watching her with that broken expression still etched on his face.
She offered him a small smile.
“Go check on her too. Come back and tell me everything about her miraculous recovery.”
This time, he didn’t hesitate.
He walked out.
Finally, she was alone.
She exhaled a long, unsteady breath and unlocked her phone, her hands shaking uncontrollably.
The system had offered her a deal once: complete the mission, and it would grant her a single wish.
She had used that wish right before they wheeled her into surgery. Two videos. That had been her request.
She pressed play. Then watched again. And again. She couldn’t make herself stop.
She watched them plot the entire thing, calm and methodical, deciding to cut her open and hand her heart away.
She watched Alessio remove her heart with steady hands and place it carefully into Gianna’s chest.
Every drop of blood in her veins turned to ice.
And in that moment, she felt almost grateful they’d revived her.
Dying would have let them off too easily.
She created an anonymous account and sent the videos to him.
For the remainder of her hospital stay, they rotated through her room one at a time, never staying long. Always claiming somewhere else to be.
She told herself it was family business pulling them away.
Then one afternoon, no nurse responded to her call button. She gripped her IV pole and pulled herself out of bed, making her way slowly down the hallway.
Passing a VIP suite, she glanced through the partially open door.
Rafe was inside. The same Rafe who’d told her he was rushing to an urgent meeting. He was carefully cutting a filet mignon into pieces and feeding it to Gianna.
Alessio, Matteo, and Dario were all there too, the exact men who claimed they couldn’t spare an hour for her. They surrounded Gianna like she was the center of their entire world.
Alessio’s voice carried warm, exasperated affection.
“Gianna, you can’t just eat whatever you want the moment you feel a little better. Steak isn’t exactly post-surgery food.”
Gianna tugged playfully at his coat sleeve, giving him an exaggerated pout.
“It’s not my fault Matteo’s steak is that incredible.”
Then she groaned, tossing her head back theatrically.
“You’re all impossible, you know that? Watching me every second of every day. I can’t even sneak a single bite without getting caught.”
Her nails dug into her palms. Something bitter tugged at the edge of her mouth.
In all her days in that hospital, not a single one of them had brought her an actual meal.
Gianna had all four men at her bedside daily, with Matteo himself, the ruthless Don, cooking for her personally.
She received whatever the hospital aide happened to grab from the cafeteria. Once, the milk had already expired.
She turned and walked away.
“Apparently the girl I’m caring for is their actual biological daughter. Can you imagine? Dr. Alessio instructed me to keep her pumped full of IV fluids. They’re prepping her to use as a living blood supply.”
She froze near the nurses’ station, catching two of them whispering.
“If Gianna’s body rejects the heart, they’re planning to drain that girl completely. A full blood exchange for Gianna, every single drop.”
Every muscle in her body locked rigid.
